Self-Pay Rates

Below you will find our self-pay rates for specific services.  These fees are only for patients wishing to be evaluated who are not filing an insurance claim.  We strive to be transparent and to provide valuable, affordable care to you.  For questions on other services, including outpatient surgery, please contact our office.

Physician Appointments

• Initial Visit/Consult $300
• Follow Up Visits $200

Occupational Therapy/Physical Therapy

• Initial Evaluation $150
• Follow-up Treatment Visits $90

MRI

• Noncontrast MRI $575
• MRI with Contrast $650
• MRI Arthrogram $950 (includes scan and professional fee for guided dye administration)

PRP (Platelet-Rich Plasma) Injections

• $650 Per Injection
